Treatment Foster Licensing

Flow Chart

PHASE I ≠ First Call For Information/Paperwork

Receive informational brochure. Then receive letter with:

∑ Foster Parent Information Sheet

∑ Releases to complete background checks

PHASE II ≠ The Meeting

∑ Before the meeting more paperwork will be sent out

o Financial Statement

o Description of Residence

o Fire Safety Plan

∑ Meeting

o Review the various paperwork

o Discuss whether treatment foster care is right for your family

o Discuss the life story which needs to be completed by each prospective foster parent

PHASE III ≠ Home Visit

∑ A block of three to four hours will be scheduled at your home

∑ We will review your life stories

∑ Complete a walk-thru of your home for compliance with statutory requirements

∑ Meet other family members

If the Foster Care Coordinator is satisfied that your family meets all the requirements and there has not been any reason to discontinue the process you will proceed to Phase IV.

PHASE IV ≠ Training

You receive a written invitation to attend pre-service training

∑ Two day training at Community Care Resources in Middleton

o 20 hours, Including homework (9AM-4PM, 2 days)

∑ Review the Foster Care Training Manual

o Treatment Planning, Referral Process

o Policy and Procedure

o Overview of Mental Health

o State Statutes governing foster care

∑ Review that all documentation is complete

o Copy of auto and homeownerπs insurance

o Pet documentation

o Copy of valid driverπs licenses

o Health check forms complete or appointments scheduled

PHASE V - Licensing

You receive your two year treatment foster license by mail and are on our active openings list to receive a child into foster placement
